Your grip strength is telling you something! If you’ve spent any time on health fitness content lately, you have probably seen the headlines about how grip strength predicts how long you will live. You can test this by squeezing a thing called a dynamometer and you’ll know all you need to know about your future!
As a PT who specializes in performance for all ages, I want to unpack that claim. Part of it is true. And the part that is true matters. But the way it is discussed misses an important step that changes how you should actually look at all this.
The research is there
It’s not all hype. Studies have shown that lower grip strength is associated with higher all cause mortality. Some studies have found that grip strength predicts mortality even stronger than blood pressure. It shows up again and again in research on cardiovascular disease, frailty, cognitive decline, and fall risk in older adults. So no, this isn’t a wellness-influencer myth. Grip strength really is one of the more useful, low-cost, two-minute measurements we have.
But here is the part that is left out: grip strength is a marker, not a mechanism. Grip strength does not make you healthy! Squeezing a stress ball a hundred times a day will make your hands stronger, it will not lower your blood pressure, protect your heart, or prevent a fall.
What grip strength actually reflects is everything happening upstream of your hands:
- Overall muscle mass and neuromuscular function. Grip strength correlates closely with total-body strength, because a strong grip requires a well-functioning nervous system efficiently recruiting muscle, not just strong forearms.
- How active you’ve been over years, not weeks. Grip strength tends to track long-term physical activity and muscle-building habits, not a single training block.
- Age-related muscle loss (sarcopenia). As we lose lean muscle mass with age, grip strength is often one of the first, easiest-to-measure signs of that decline, which is exactly why it’s become a standard part of geriatric assessments.
So, grip strength is a proxy. It’s a convenient window into a much bigger picture, your muscle mass, your activity level, your neuromuscular health, your training history. It’s useful precisely because it’s easy to measure, not because your hands are doing the protective work themselves.
What to do with this information:
Grip strength is a very useful screening tool. If yours is low, that’s worth paying attention to. But the response to a low number isn’t to squeeze more. It’s asking yourself or your provider what is driving that. Is it overall muscle mass? Activity level? A neurological factor? That is where you will find the most helpful intervention.
Train the whole system- strength, balance, cardiovascular fitness, mobility- and your grip strength will very likely follow along as a byproduct. Chase the number directly, and you’ll build a strong handshake without necessarily building the health it’s supposed to represent.
